Information clause used by Military University of Technology
In accordance with Art. 13 of Regulation 2016/679 of the European Parliament and of the Council (EU) of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC, we inform you that:
1) The Personal Data Controller of your personal data is the Military University of Technology im.Jarosława Dąbrowskiego, ul. Gen. Sylwestra Kaliskiego 2, 00-908 Warsaw.
2) The Personal Data Controller appointed the data protection officer supervising the correctness of personal data processing, who can be contacted by email: iod@wat.edu.pl
3) Your personal data will be processed for the conclusion and execution of the contract on logistical support for the organization of the international conference “24th Conference on Liquid Crystals” or participation in the conference:
a) in case of a person designated to conclude a contract according to Article 6(1)(b) of the Regulation,
b) in case of a person designated to contact/supervision/performance of the contract according to Article 6(1)(f) of the Regulation,
c) in case of participants of the Conference according to Article 6(1)(a) of the Regulation,
d) for the assertion and defense of claims according to Article 6(1)(c) and (e) of the Regulation.
4) Providing personal data of people designated for the conclusion and preparation of the contract is a contractual requirement and is a condition for its conclusion. In the case of participants providing personal data is voluntary but necessary for the performance of the contract.
5) Your personal data (name and surname) will be available to the hotel where Conference participants will be accommodated.
6) You have the right to demand access to your personal data from the Data Controller, as well as the right to correct, delete or limit the processing of the data.
7) You have the right to access personal data and, based on applicable law: the right to request rectification of personal data, the right to request the deletion of personal data (the so-called right to be forgotten), the right to request the restriction of the processing of personal data, the right to transfer personal data, the right to object to the processing of personal data, the right to withdraw consent to the processing of personal data, at any time without affecting the lawfulness of the processing carried out based on consent before its withdrawal.
8) You have the right to submit a complaint to the relevant supervisory body (President of the Personal Data Protection Office).
